Washington State Healthy Youth Survey 2025

Our school is participating in the Washington State Healthy Youth Survey during fall 2025. The survey will be given to students in Grades 6, 8, 10 and 12 in large districts and students in Grades 6 through 12 in small school districts. The Healthy Youth Survey includes questions related to physical activity, unintentional and intentional injury, substance use, interpersonal relationships, school experiences, risk and protective factors, and access to school-based services. Survey results are used by schools, communities, and State agencies to plan programs to support our youth and reduce their risks. Participation in the survey is voluntary and steps are taken to ensure student privacy. There are no names or codes used to link a survey back to a student.

Parent/Guardian Complaint Process

Innovation High School (IHS) is committed to resolving concerns fairly and promptly, in alignment with our core values. As required by RCW 28A.710.187, we have both informal and formal complaint processes in place.

Informal Process:
Start by addressing concerns directly with the person involved. If unresolved, contact the Executive Director for mediation. We ask that informal steps be followed before escalating further.

Formal Process:
If the issue is not resolved informally, submit a signed letter to the Executive Director describing the issue, the policy or procedure violated, and efforts made to resolve it. A resolution team will respond within 5 workdays.

Board Involvement:
If you are not satisfied with the outcome, you may contact the IHS Board Chair or Vice Chair for further review.

For more details, please refer to the Student Handbook.

IHS Charter School Authorizer:

To file a complaint about an SPS-authorized charter public school, please email the SPS Charter Schools Director. All charter public schools have their own complaint processes which should be followed first before filing a complaint with the charter authorizer.
